Our Admission Department including a clinical admission director, clinical patient care managers, and a team of experienced intake coordinators provide same-day admissions, everyday with quick response time.

There When and Where You Need Us

· Same-day admissions, all day, every day including weekends and holidays. 

· Quick response time within two hours to 24 hours of referral.

· Weekend patient visits

· Admission follow up visit day after admission

· Customized care plans based on individual needs

· Providing care to wherever patient calls home.

· Services help to prevent re-hospitalizations

Medication, Equipment and Treatment Support

· IV Hydration

· Antibiotics

· IV cardiac medications

·  – Milrinone, Dobutamine, Lasix

· Morphine / Dilaudid pump

· High flow oxygen  –  BiPAP, CPAP

· TPN – PPN - palliative parenteral

· Extensive wound care including Wound Vac

· Pleurex catheter

· Paracentesis

· PEG tube

Additional Therapies and Programs

· Physical Therapy

· Occupational Therapy

· Speech Therapy

· Music Therapy

· Massage Therapy

· Veteran Services Coordinator for military patients

· Trained Hospice Volunteers provide companionship and specialize visits such as pet therapy, bedside art and crafts.

· Angelic Health Foundation supports costs not covered by insurance and grants for final wishes.

· Long-term bereavement support for family

Hospice care is covered by Medicare, Medicaid, and most private insurances. 

Treatment options must be medically necessary for pain and symptom management as part of the patient care plan. The clinical appropriateness of any treatment is determined by the physician in  consultation with the hospice interdisciplinary team.
